2009 "Math Mania" Speaker Proposal Form

    This form is used to submit a proposal to speak at the annual conference.  Our next conference, "Math Mania", will be held March 20 - 21, 2009 at the Flatwoods/Sutton Days Inn Hotel Conference Center.  Proposal forms are due October 10, 2008.

2009 "Math Mania" Exhibitor Form

    This form is used to exhibit at the annual conference.  Our next conference, "Math Mania", will be held March 20 - 21, 2009 at the Flatwoods/Sutton Days Inn Hotel Conference Center.  Exhibitor forms are due October 15, 2008.

2009 T3 Regional Conference Speaker Proposal Form

    This form is used to submit a proposal to speak at the T3 Regional Conference.  The T3 Regional Conference will be held March 21 - 22, 2009 at the Flatwoods/Sutton Days Inn Hotel Conference Center. 

Membership Form

    This form is used to join the West Virginia Council of Teachers of Mathematics.

2009 Distinguished Service Award

    This form is used to nominate an outstanding West Virginia Educator for WVCTM's highest honor, The Distinguished Service Award, otherwise known as "The Chair".  Nominations are due February 1, 2009.

2009 Teacher of the Year Awards

    This form is used to nominate an outstanding West Virginia Educator to be the 2009 WVCTM Mathematics Teacher of the Year.  Nominations are due February 1, 2009.

2009 Teacher-In-Training Grant

    This form is used to apply for the 2009 Walter Regula Teacher-In-Training Grant.  This is a $600.00 grant awarded to an aspiring Mathematics Teacher.  The application is due February 1, 2009.

2009 Golden Holtan Grant

    This form is used to apply for the 2009 Golden Holtan Grants.  WVCTM awards up to five $400.00 mini-grants to be used in West Virginia classrooms.  The application is due February 1, 2009.
